Youth Our Way Forward Intervention

MENTORING, DIVERSION & PATHWAYS

Youth Mentoring, Justice Diversion & Pathways

Early intervention and culturally informed mentoring programs that keep young people 10 – 17 years out of the justice system by supporting prosocial behaviour and stronger community connections. Helping young people to re-engage in education and vocational pathways, including re-engagement with school, TAFE, apprenticeships, or employment opportunities. By combining mentoring and diversion with education and training, FACESS provides holistic support that addresses both immediate risks and long-term opportunities – helping young people make positive choices, build skills and create sustainable futures.

Strengthening Services

  • Provide short and long-term case management to young people and their families.

  • Deliver specialised parent and family trainings and workshops to restore family relationships.

  • Provide information and referral service support to prevent community access barriers.

  • Provide in-home support to youth and their families to deliver critical service supports, advocacy and guidance.

  • Assisting youth to access job readiness and education support programs.

  • Proactively engage youth to stay on track in education and vocational pathway trainings.

  • Provide outreach services to reach at risk youth to ensure ongoing support is accessible.

  • Provide peer support workshops to empower behavioural change.

  • Facilitate individual and group support workshops and counselling.

Empowering Independence

FOSTERING AUTONOMY

FACESS Transition to Adulthood supports young people leaving care by helping to build and develop essential life skills, engage in education and vocational pathways, reduce homelessness and reconnect with needed service providers to achieve independence. Through early and targeted support, the program helps young people navigate challenges, build confidence, and create a stable foundation for their future.

Our FACESS Team provides culturally safe case management, working alongside stakeholders, community service providers and families to ensure young people receive crucial and needed supports.

FACESS tailored support include:

  • Case management to connect young people with essential services and support networks.
  • Strengthening family relationships and accessing local community services.
  • Education, training, employment preparation, and life skills development, including assistance with securing identification documents and job readiness.

FACESS is committed to equipping young people with the skills, knowledge, and support needed for long-term success in education, employment, life skills and building their independence.

  • Tailored support to help young people secure safe, stable housing.

  • Assistance in building strong family and community connections to foster a sense of belonging and support.

  • Develop essential life skills to navigate building independence with confidence.

  • Access to educational and vocational training pathways, empowering young people to pursue their goals.

  • Information, referral, and advocacy services to ensure young people are connected to the right resources.

  • Targeted in-house support, helping youth to maintain a stable living environment.

  • Outreach support services that provide ongoing assistance and ensure youth have access to the care and guidance they need.

Cultural Safe Care and Connect

Strengthening Family and Kinship Systems

Cultural Safe Care and Connect (CSCC) is a unique First Nations-led practice framework developed by Family and Childrens Emergency Support Services Founders. It is designed to uphold the cultural identity, safety and belonging of Aboriginal and Torres Strait Islander children, young people and families – especially those involved in out-of-home care, youth justice or at risk of disconnection from kin, culture and community.

CSCC is more than just a cultural plan – it is a relational model of practice that embeds cultural authority, family leadership, young people voice and self-determination into every stage of support.

Family Support and Advocacy

  • Restore and maintain cultural identity for young people in care or at risk

  • Facilitate safe reunification with family and kin

  • Support transition out of care into adulthood with culturally guided life planning

  • Reconnect young people to education, training and community

  • Strengthening kinship systems and prevent disconnection
